BURNETT COUNTY -- Burnett County Department of Health and Human Services-Public Health and Burnett Medical Center are assessing Burnett County’s health. There will be an online survey available for you to share your views and opinions to help us understand the health needs of Burnett County so that we, in partnership with other community stakeholders, can create an action plan to improve our county’s health and well-being. If you live OR work in Burnett County, you are eligible to take this survey. Your responses will be anonymous and confidential.
The survey will be available October 4th, 2021 through November 1st, 2021.
