Meet Renee Brown, Administrator of Transitions at Home North

NORTHERN WISCONSIN — Transitions At Home, a nonprofit home health agency dedicated to serving elderly and disabled individuals through quality healthcare and supportive services, is pleased to introduce its new Administrator, Renee Brown.

Brown joined Transitions at Home in October 2025 and quickly adapted to her role as Administrator. While new to home health, she is well known throughout the community for her compassionate leadership, empathetic approach, and commitment to serving others.

"The organization's core values of doing the right thing, fostering positive relationships, being committed, being innovative, and coaching, guiding, and leading are what drew me to this role, as they align perfectly with both my personal and professional values," said Brown.

Brown works alongside Clinical Manager and Registered Nurse Jodi Effertz, who has been with Transitions at Home for five years and has dedicated her career to providing compassionate and patient-centered care. "Home healthcare allows us to meet patients where they are and provide care that supports their independence, dignity, and quality of life," said Effertz.

Together, Brown and Effertz lead a team committed to bringing high-quality home healthcare services to rural communities throughout Northern Wisconsin (Washburn, Sawyer, Douglas, Burnett, Barron, Rusk, and Bayfield Counties). Services include skilled nursing, physical therapy, occupational therapy, home health aide services, and non- medical caregiving support.

The Transitions at Home North team continues to strengthen its presence across Northern Wisconsin while expanding access to quality home healthcare services. Browns focus on collaboration, community partnerships, and patient-centered care to support the agency's mission of helping individuals remain safe, independent, and comfortable in their own homes.

Transitions at Home is currently seeking a Home Health Physical Therapist Assistant (PTA) to join their growing team.
