Voice Your Opinions About Community Health Needs

Hayward Area Memorial Hospital (HAMH) is working with Sawyer County to conduct each organizations’ Community Health Needs Assessment (CHNA). Every three years, hospitals are required to conduct a Community Health Needs Assessment (CHNA) and every five years counties are required to complete a CHNA and Community Health Improvement Plan (CHIP).

HAMH is in the early stages of conducting their 2019-2021 CHNA. For our next plan we are working closely with Sawyer County Health & Human Services, Water’s Edge and many other local agencies. We are currently in the assessment stage and we are seeking community support by asking residents to complete a short survey.

The survey is based on the Wisconsin State Health Plan, Healthiest Wisconsin 2020. A small portion of the survey is also gathering information on an area of focus from our current CHNA, Access to Care.

“Our community faces a large number of complex issues in providing high quality healthcare and other community services,” said Luke Beirl, CEO of Hayward Area Memorial Hospital & Water’s Edge. “Fortunately, the hospital, Water’s Edge and Sawyer County are working together to engage community members, organizations and service providers to create a plan to address our greatest needs.”

The survey is anonymous. However, those willing to provide their name and address will be entered into a drawing for Hayward Chamber Dollars. The names and addresses will only be used for the purpose of providing the prize. Paper copies of the survey are available in the hospital lobby, library or Sawyer County Health Department.
